987 Inclusion Health

NCL Wide

987 provides a pathway for patients to attend the Ambulatory Emergency Care Unit at University College Hospital for assessment by the Inclusion Health Team, which includes a medical consultant, senior nurse, clinical nurse specialists, and a senior support worker, specialising in unmet medical and social needs and onward referrals to appropriate services.


Eligibility Criteria

Inclusions

IH patients, meaning over 18s who:

  •  are homeless
  •  use, or have used, class A drugs
  •  are on methadone/buprenorphine
  •  struggle with persistent alcohol misuse
  •  are undocumented migrants
  •  have other issues that mean they may have been excluded from good quality healthcare in the past.

How to Refer

To access 987 contact the IHT directly via email with the completed referral form. James and the team will then book the patient into AECU and confirm the time and date for the patient to attend.
